Much of this change is the result of demographic shifts in the population curve, changes in public policy, and legislative changes in many countries [18]. The increasing prevalence of injection drug use has contributed to a higher incidence of DVT in young patients and its association is undeniable [11]. However, this association may be overlooked by physicians, and intravenous drug use is not even included in the Wells criteria, the most commonly used tool to screen patients for DVT. Although research is challenging, more studies are needed to gain a better understanding of the problem. Broader knowledge will lead to the development of more appropriate preventive measures, diagnostic approaches, and treatment protocols [9,15]. There are over 50 conditions that present with swollen and painful limbs comparable to DVT in IVDU, making precise diagnosis critical for timely treatment.

Hepatitis C

Skin and soft-tissue infections and complications of bacteraemia remain common, but outbreaks of rare infections do occur, so it is essential to remain alert for severe or unusual presentations. Identification of IDUs is crucial – both to determine clinical risk and to minimise the risk of unexpected withdrawal. An accurate drug history, including route of administration, should be iv drug use completed for all patients on admission, acknowledging that this may need to be repeated until an accurate picture is gained. All IDUs should be asked about the use of other substances, especially alcohol and benzodiazepines. For patients on opioid maintenance therapy, the dose and timing of the last dose should be confirmed with the dispensing centre at the first opportunity.

Supplementary Data

PWID interviewed in this study demonstrate what is likely widespread basic understanding of safe injection practices. In addition, PWID interviewed noted information about “safe” practices often travels via word of mouth rather than from medical professionals. PWID would benefit from pervasive messaging throughout the medical system, provided through a more complex and in depth understanding of the potential risks and prevention strategies.

Syphilis and Other Sexually Transmitted Diseases

It is important to remember that major physical events and near misses — such as spells in hospitals with SABE — can provide the opportunity for a patient to re-appraise their life and provide an opportunity to move forward positively. And, as for all patients, non-medical life events rather than interventions from medical professionals can provide the opportunity for change. Striking a balance between realism and being positive about change (a necessary attribute for positive therapeutic effect) provides one of the challenging necessary skills for the GP and other members of the primary healthcare team.
